As a referring doctor, you will be informed at every step in the process with written documentation after every patient visit. Our goal is to provide you and your patients with the highest quality diagnostic and treatment capabilities to improve or retain vision – and then to return the satisfied patient to you.

How to refer a patient

Referring a patient is easy. Simply print out a copy of IEI’s patient referral form and fax the completed version to 312/949-7347. We also invite doctors to tour IEI’s facility with one of our specialists.
